Job Title: Patient Navigator RN

Join Northern Light Health as a Patient Navigator RN and play a vital role in delivering exceptional patient care.

About the Role:

We are seeking a skilled and compassionate Registered Nurse to serve as a Patient Navigator RN. In this role, you will provide nursing care in physician practices, working closely with clinical coordinators, RN practice managers, clinical supervisors, and the Director of Clinical Services.

Key Responsibilities:
  • Provide patient navigation through scheduled pre-operative testing and post-operative care at home.
  • Manage all Structural Heart Program registries.
  • Referral management and patient triage calls.
  • Collect and analyze Structural Heart statistics.
  • Communicate effectively with patients, families, and healthcare teams.
  • Assist departments with questions and provide excellent customer service.
  • Participate in staff education and training as needed.
  • Perform administrative tasks to support department operations.
  • Follow established infection control procedures.
  • Lead work efforts of subordinates and peers.
  • Meet patients' psychological, emotional, or physical needs.
  • Share knowledge and serve as a resource in a team environment.
Requirements:
  • Registered Nurse (RN) licensure.
  • Bachelor's Degree in Nursing.
