Job Summary:

Interim HealthCare is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health Clinical Support Specialist. As a key member of our team, you will be responsible for providing exceptional patient care and support to our clients in the comfort of their own homes.

Responsibilities:

Act as the primary point of contact for referral sources, outside agencies, and health professionals
Receive and coordinate incoming calls, providing patients and referral sources with basic information to access services
Communicate with patients and families regarding arrangements for care and services
Collect and enter customer information into our management information system
Work in conjunction with clinical staff to adhere to standards of practice for nursing and applicable home care laws and regulations

Requirements:

Graduate of an accredited LPN program and active LPN licensure in Colorado
Minimum of one year of LPN experience, preferably in home healthcare
CPR certification and QAPI experience preferred
Ability to pass a background check and drug test
Knowledge of state and federal home health regulations
Strong clinical proficiency in nursing, excellent communication skills, and compassion
