Hospice of Surry County is seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a RN Case Manager, you will provide expert nursing care and emotional support to patients and their families in their homes and healthcare facilities.

**Key Responsibilities:**

* Provide holistic assessments to understand and address patients' physical, emotional, and spiritual needs
* Develop tailored care plans in collaboration with patients and their loved ones
* Coordinate services and resources to optimize patient care and support
* Educate patients and families about hospice services, disease progression, and end-of-life care options
* Administer medications, manage symptoms, and adhere to clinical protocols with precision and empathy
