Job Summary

The Advanced Practice Provider (APP) at American Family Care Chino Hills will work collaboratively with other providers and staff to ensure efficient patient flow and high patient satisfaction. This role requires a strong communicator with excellent interpersonal skills.

Responsibilities
  • Conduct thorough health histories, review patient records, and develop comprehensive medical assessments.
  • Order laboratory, radiological, and diagnostic studies as necessary.
  • Formulate medical and nursing diagnoses and implement treatment plans.
  • Provide instructions and guidance on healthcare and health promotion to patients and their families.
Qualifications
  • Excellent communication and interpersonal skills.
  • Professional appearance and attitude.
  • ANCC, AANP, or NCCPA Board Certification.
  • Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice.

Able to work independently and multitask in a fast-paced environment.
