Community Health Nurse Specialist

NYC Health + Hospitals/Community Care is seeking a highly skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals managing chronic health conditions in Brooklyn, Manhattan, Queens, and The Bronx.
Job Summary:Under the direction of the supervising Assistant Director of Nursing, you will assess, plan, implement, and evaluate patient care plans. You will provide direct skilled nursing care, observe and instruct patients in their place of residence, and may coordinate services with other caregivers.
Key Responsibilities:- Provides individualized skilled nursing care to patients requiring part-time intermittent professional nursing service.
- Conducts assessments of patients and develops plans of treatment and care with ongoing evaluation of patient/family needs for other services.
- Provides professional nursing services as outlined in the patient's plan of treatment and authorized by the patient's physician, and provides emergency treatment as needed.
- Records physical, social, psychosocial, and environmental needs of the patient, and provides ongoing monitoring of the patient in the home care setting.
- Escalates any clinical changes to the physician or non-physician provider as needed.
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
- One (1) year of experience as a registered nurse.
- Ability to travel regularly to patient/client assignments across New York City via public transportation and/or private vehicle.
- Demonstrated commitment to continued professional development.
- Minimum two (2) years of full-time nursing experience.
- At least one (1) year of experience in a hospital setting.
- At least one (1) year of experience working in a Labor-Delivery, Neonatal Intensive Care Unit (NICU), or postpartum care setting.
