As a Community Health Nurse, you will be responsible for providing high-quality patient care in the comfort of patients' homes. Your expertise will be invaluable in assessing, planning, implementing, and evaluating patient care plans.
Key Responsibilities:
- Provide individualized skilled nursing care to patients requiring part-time intermittent professional nursing service.
- Assess patients and develop plans of treatment and care with ongoing evaluation of patient/family needs for other services.
- Provide professional nursing services as outlined in the patient's plan of treatment and authorized by the patient's physician.
- Record physical, social, psychosocial, and environmental needs of patients.
- Provide ongoing monitoring of patients in the home care setting and escalate any clinical changes to the physician or non-physician provider as needed.
Requirements:
- Licensed and currently registered to practice as a professional nurse in New York State.
- Current certification in Basic Life Support procedures, including CPR.
- Baccalaureate Degree in Nursing from an approved program preferred.
