- Community Resource Referrals: Provide referrals for services to community agencies as appropriate, ensuring patients/families receive necessary support.
- Education and Support: Provide general information to individuals and families on program objectives and services, eligibility requirements, and benefits.
- Relationship Building: Establish trusting relationships with patients and their families, providing general support and encouragement.
- Ongoing Follow-up: Provide ongoing follow-up with patients via phone calls, home visits, and visits to other settings.
- Care Coordination: Work closely with medical providers to ensure patients have comprehensive and coordinated care.
- Communication: Provide consistent communication to the Care Management Team.
- Record Keeping: Record patient care management information in the EMR and other software within 24 hours of patient contact.
- Community Resource Development: Continuously expand knowledge and understanding of community resources and services, facilitating client access to relevant services.
- Liaison Services: Serve as a liaison between clients and community resources, including department staff, City, State, and Federal social services agencies.
- Event Planning: Assist with planning, organizing, and implementing community special events, such as health fairs and workshops.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. A strong understanding of community resources and services is essential, as is the ability to navigate complex healthcare systems.
