Job Summary:

Assist patients with daily activities, provide personal care, and support their overall well-being under the direction of a registered nurse (RN) and according to the patient's Plan of Care (POC).

Main Responsibilities:

  • Provide direct patient care, including bathing, grooming, skin care, shaving, oral care, nail/foot care, and other activities
  • Assist with feeding patients, communicating basic principles of nutrition, and observing food and fluid intake when necessary
  • Help patients maintain good personal hygiene and mobility, exercising, positioning/turning, transfers, and ambulation per the Plan of Care and CenterWell Home Health policy
  • Document interactions with patients, caregivers, doctors, and other staff members accurately, legibly, thoroughly, and within the allotted time
  • Practice acceptable infection control principles and provide a clean, safe, and comfortable environment

Requirements:

  • High school diploma or equivalent
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months
  • Must meet applicable state certification requirements
  • A valid driver's license, auto insurance, and reliable transportation are required
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency
