Senior Care Manager

We are seeking a highly skilled and compassionate Senior Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing exceptional care and services to our residents, ensuring their physical, emotional, and social needs are met.
Key Responsibilities- Administer medications and treatments in a safe and organized manner, adhering to established protocols and procedures.
- Conduct daily medication passes, ensuring accurate and timely administration of medications.
- Document resident medication administration, including any refusals or missed doses.
- Collaborate with the Resident Care Director and Wellness Nurse to ensure accurate and up-to-date medication information.
- Participate in daily Cross Over meetings to discuss resident care and concerns.
- Conduct Service and Health Updates as directed by the Resident Care Director.
- Develop and implement Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs) in collaboration with the care team.
- Valid Ohio LPN or RN licensure.
- CPR and First Aid certification.
- Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with the care team and residents.
- Ability to maintain confidentiality and handle sensitive resident information.
- Flexibility to work varied shifts, including evenings and weekends.
