Job Summary: The Healthcare Services Coordinator plays a crucial role in ensuring seamless transitions of care for patients at Intermountain Health. This position requires strong organizational and communication skills, as well as a deep understanding of healthcare services and regulations.

Key Responsibilities:
  • Coordinate patient care, including referrals, authorizations, and follow-up appointments.
  • Evaluate patient information to identify care gaps and develop targeted interventions.
  • Develop and maintain relationships with community resources to ensure efficient transitions of care.

Education and Experience:
  • Graduate of an accredited Registered/Professional Nursing program.
  • State licensure as a Registered Nurse (RN).
  • Bachelor's degree in Nursing or relevant certification as determined at position level.
  • 2 years' nursing experience in outpatient care and 1 year Care Management experience.

Compensation:$45.49 - $67.65 per hour, depending on experience.
