  • NYC Health + Hospitals/Gouverneur (MANHATTAN) provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly and infirmed in its 295-bed Skilled Nursing Facility (SNF).
  • NYC Health + Hospitals/McKinney (BROOKLYN) provides high-quality short-term rehabilitation and long-term skilled nursing care in a comfortable, person-centered setting.
Job Description:Key Responsibilities:
  1. Assist RNs in admission procedures and participate in planning, implementing, and evaluating nursing care.
  2. Set priorities and deliver direct care to meet the needs of assigned residents.
  3. Provide medications, treatments, and perform nursing procedures safely, documenting per policy and procedure in resident medical records.
  4. Participate in discharge planning and provide necessary teaching to residents and families on a regular basis.
  5. Demonstrate safety awareness and follow protocol for fire, disaster, infection control, and other environment of care practices.
  6. Maintain competency and proficiency in nursing resident care standards, attending mandatory classes and participating in continuing education programs and other learning experiences.
  7. Participate in performance improvement activities and other committees as assigned.
  8. Perform other related duties.
  9. Supervise Nursing Aides as directed and assigned.
Requirements:
  • Currently licensed or otherwise duly authorized to practice as a Licensed Practical Nurse in the State of New York.
