As a key member of the High Desert PACE team, the Enrollment Registered Nurse RN plays a vital role in determining the eligibility of prospective participants for our program. Working closely with the Business Development and Enrollment Development Department, this individual will assess the level of care required for each participant, ensuring they meet the necessary guidelines for admission.
Responsibilities- Conduct home visits and/or center-based clinic appointments to assess the level of care required for prospective participants.
- Prepare documentation from home visits and submit to the CA State Level-of-Care (LOC) nurse for review.
- Track and oversee the annual State level-of-care review process.
- Collaborate with the interdisciplinary team (IDT) and outreach & enrollment (O&E) team to ensure seamless communication and care coordination.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Complete eligibility referral documents and compile supporting documentation for application to the State for enrollment.
- Associate degree from an accredited nursing school.
- Current Registered Nurses License in the state in which working.
- Current CPR certification or the ability to obtain CPR certification.
- 3 years health care experience with emphasis in geriatrics.
- 1 year working with the frail or elderly.
