We are seeking highly skilled and compassionate physical therapists to join our team. Our Home Health Physical Therapists work as part of a multidisciplinary team to provide personalized care to patients in their homes.

About the Job:

  • Provide home-based physical therapy services to patients who require assistance with mobility, strength, and functionality due to illness or injury
  • Work collaboratively with a healthcare team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans
  • Conduct assessments, observe patient movements, document progress, and adjust care plans as necessary
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their care plans, established goals, exercises, and self-care strategies
  • Counsel family members on care plans, therapy objectives, and safely assisting patients with exercises

Requirements:

  • Graduate of an accredited Physical Therapist Program and active PT license in Colorado
  • Minimum 1 year of experience in physical therapy, preferably in home healthcare
  • BLS/CPR certification (demonstration course required)
  • Familiarity with state and federal regulations governing home health care
  • Strong clinical judgment, interpersonal skills, dexterity, and compassion
  • Pass a background check and drug test (required for THC detection)

Estimated Salary Range: $97k - $120k per year, based on location and experience
