About the Role

We are seeking a skilled Nurse Practitioner to join our HouseCalls team at Optum. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Conduct comprehensive in-home assessments, including medical history reviews, medication reconciliation, and physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses and develop care management plans to address patient needs.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices.
Requirements
  • Active NP license or ability to obtain by start date.
  • APCs must have approval to practice autonomously or without formal supervision, if applicable.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain certification by start date.
  • Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling for physical assessments.
Preferred Qualifications
  • 1+ years of clinical experience in a related setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel up to 5% of the time, with advanced notice, for additional compensation incentives.
