We are seeking a skilled and compassionate RN to join our team as a Pediatric Case Manager. As a key member of our care team, you will be responsible for providing high-quality care to our pediatric patients and their families.
Responsibilities:- Manage client care plans and ensure timely and accurate documentation.
- Conduct home and hospital visits to assess patient needs and provide education to families.
- Collaborate with the clinical team to develop and implement individualized care plans.
- Provide training and supervision to new staff members.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Active unrestricted Registered Nurse (RN) license.
- At least one year of experience as an RN within the last 36 months.
- Ability to prioritize tasks and manage multiple responsibilities.
- Excellent communication and interpersonal skills.
