Job Summary

We are seeking a highly skilled Care Worker UM, HMSA to join our team at Magellan Health Services. As a key member of our clinical team, you will play a vital role in facilitating department workflow and providing administrative care coordination support.

Key Responsibilities
  • Support clinical teams with administrative care coordination tasks, ensuring seamless workflow and efficient communication with hospitals and providers.
  • Locate and connect members with community services and referral locations, arranging access to care and facilitating communication with healthcare providers.
  • Provide non-clinical support to workflows involving utilization management, case management, disease management, and health promotion.
  • Troubleshoot and resolve issues related to provider availability, service types, and care levels.
  • Stay up-to-date with community resources and services, linking members to appropriate care and support.
  • Assist with transitions in care for hospitalized patients, making informed placements as needed.
  • Develop and maintain knowledge of plan benefit structures, psychiatric/medical terminology, and local communication resources.
  • Participate in operational activities, including data collection, tracking, and analysis.
