Job Title: Nurse Practitioner Specialist

We are seeking a skilled Nurse Practitioner Specialist to join our team at Liberty Health System.

  • About the Role:
  • This is a full-time position working days, with occasional on-call responsibilities.

The successful candidate will be responsible for providing high-quality nursing care to patients in local Skilled Nursing & Rehabilitation facilities. They will assure quality nursing care by directing and coordinating nursing care, conducting physicals and annual assessments, and completing acute visits to assess and diagnose patient conditions.

Key Responsibilities:

  • Provide client services to local Skilled Nursing & Rehabilitation facilities
  • Assure quality nursing care by directing and coordinating nursing care
  • Conduct physicals and annual assessments for assigned patients
  • Complete acute visits to assess and diagnose patient conditions
  • Focus on early identification of changes in condition as well as strong management of chronic conditions
  • Complete history and physical on new admissions within 72 hours of admission or readmission
  • Complete Medicare certifications according to required time frame
  • Prepare discharge orders, instructions, and prescriptions for patients being discharged
  • Encourage advanced care planning and end-of-life decisions for patients requiring these services
  • Provide pharmacy with needed narcotic prescription refills as patient need dictates
  • Document patient assessment and plan of care in the electronic medical record
  • Review labs and other diagnostic tests for additional interventions
  • Collaborate with physicians and staff
  • Notify family of changes in condition
  • Communicate daily with nursing staff, assign duties, supervise, and evaluate work performance
  • Review nursing policies and procedures for compliance and make recommendations for changes/additions as necessary
  • Recommend or conduct specific areas of in-service or continuing education
  • Assess patients for palliative care needs and create/oversee the palliative plan of care in facilities and in patients' homes

Requirements:

  • Current, unrestricted valid Nurse Practitioner license from the Board of Nursing/PA License from the Medical Board in the state of assignment or the ability to obtain
  • DEA certificate current or the ability to obtain
  • Clinical background in adult, family, or geriatric specialties preferred
  • Past experience working in long-term care, a nursing home, or with seniors in other settings preferred
  • Strong communication and relationship-building skills preferred
  • Intermediate level of proficiency in PC-based word processing, including Microsoft Word and Outlook
  • Willingness to be 'on call' for Facilities as needed
  • Dependable, flexible, and able to work and cooperate well with all nursing personnel and other departments
  • Ability to make decisions regarding nursing problems, work well under pressure, and problem-solve

Compensation:

The estimated salary range for this role is $80,000 - $120,000 per year, depending on experience.
