Position Overview:

This role involves delivering nutritional services and education to patients undergoing dialysis and their authorized caregivers within a clinical environment. The aim is to enhance and support the nutritional health of patients with end-stage renal disease, adhering to the policies and guidelines set forth by LifeBridge Health, as well as relevant regulatory standards.

Key Responsibilities:
  • Conduct comprehensive assessments of nutritional status to establish suitable nutrition interventions and recommendations.
  • Perform initial assessments within a specified timeframe or upon patient admission to the facility.
  • Engage in re-assessments periodically based on patient stability and needs.
  • Collaborate in the development of interdisciplinary care plans, ensuring timely participation following assessments.
  • Review and document medical and dietary histories, maintaining accurate records of nutritional progress.
  • Create tailored Medical Nutrition Therapy plans, including individualized meal plans for each patient.
  • Provide nutrition education and counseling, considering each patient's unique circumstances and requirements.
  • Monitor and evaluate nutritional status, laboratory results, and adherence to dietary recommendations.
  • Revise nutritional care plans as necessary based on ongoing evaluations.
  • Facilitate communication with patients, caregivers, and the healthcare team regarding nutrition-related issues.
  • Participate in quality improvement initiatives and patient care conferences.

Qualifications:
  • Bachelor's degree in Nutrition, Dietetics, or a related field.
  • Registered Dietitian credentialing from the Commission on Dietetic Registration.
  • State licensure as required.
  • A minimum of two years of experience in clinical nutrition, with a preference for experience in dialysis settings.
  • Strong communication skills, both verbal and written, along with excellent time management abilities.
  • Ability to work collaboratively within a multidisciplinary team.
