Key Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health will:

  • Conduct thorough assessments and evaluations of patient care needs, focusing on functional status, mobility, endurance, pain management, and other medical conditions.
  • Collaborate with healthcare professionals to develop effective treatment plans.
  • Implement interventions aimed at enhancing the patient's overall well-being and monitor their progress.
  • Evaluate patients' functional capabilities and therapy requirements.
  • Assist physicians in formulating therapy plans.
  • Document and communicate patients' responses to treatment and any changes in their conditions to the supervising nurse and/or physician.
  • Provide treatment to alleviate pain, restore function, and maintain optimal physical performance using established standards of practice.
  • Educate patients, families, and healthcare team members about the care plan, functional limitations, and progress towards goals.
Licensure Requirements
  • Current Physical Therapy licensure in the state of New Jersey
  • Current CPR certification
  • Possession of a valid driver's license and vehicle insurance, along with access to a reliable vehicle
