About the Role

This role involves providing comprehensive primary care services to patients in our outpatient settings. As an Advanced Practice Primary Care Provider, you will work collaboratively with physicians and other healthcare professionals to deliver exceptional patient care. Your responsibilities will include conducting office visits, administering annual wellness visits and health risk assessments, and coordinating care with other providers and specialists.

Key Responsibilities
  • Provide primary care services to patients, including routine and acute care.
  • Administer annual wellness visits and health risk assessments.
  • Care coordination with other providers and specialists.
  • Population health leadership and management.
  • Educate patients on their health conditions, care plans, and treatments.
Requirements
  • Master's degree in Nursing or related field.
  • National certification as a Nurse Practitioner through AANP or ANCC.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.
Preferred Qualifications
  • Experience in primary care: internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
Salary and Benefits

We offer a competitive salary range of $120,000 - $180,000 per year, depending on experience, plus benefits including:

  • Quarterly bonus based on quality metrics.
  • 6 weeks of paid time off, inclusive of vacation, sick time, and major holidays.
  • $5,000 CME stipend.
  • Tuition reimbursement.
  • Provided Health, Vision, Dental, and Life Insurance.
  • 401K Investment, up to 4% company match, vested immediately.
