We are seeking an Hospice Clinical Social Worker to join our team at Farmington CT HOS. As a key member of our interdisciplinary care team, you will work closely with patients, families, and healthcare professionals to provide comprehensive social work services.

Key Responsibilities:

  • Conduct thorough assessments of patients' social, emotional, and environmental needs
  • Develop and implement individualized care plans to address patient goals and priorities
  • Provide counseling, education, and advocacy to patients and their families
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Maintain accurate and timely documentation of patient interactions and progress

Requirements:

  • Bachelor's degree in Social Work or related field; Master's degree preferred
  • Licensed clinical social worker (LCSW) in the state of Connecticut
  • Minimum 2 years of experience in hospice or palliative care
