The Registered Nurse - Home Care Specialist is a key member of our team at Capital Health Care Network, responsible for planning, organizing, and directing home care services. This role requires a skilled nurse with experience in community health education and a passion for delivering high-quality patient care.
Key Responsibilities- Patient Care
- Conduct thorough initial assessments of patients and families to determine home care needs.
- Develop and implement individualized care plans that address patient needs and goals.
- Provide education and support to patients and families on health management and disease prevention.
- Administer medications and treatments as prescribed by physicians.
- Communication
- Collaborate with healthcare teams to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and compliance with care plans.
- Document patient progress and communicate changes to healthcare teams.
- Additional Duties
- Participate in on-call duties to ensure 24/7 patient care.
- Coordinate with community health resources to ensure patients receive necessary services.
- Supervise and evaluate home health aide care provided to patients.
- Current licensure as a Registered Nurse through the State Board of Nursing.
- Valid driver's license and reliable transportation.
- At least one year of general nursing experience, preferably in home health, clinical, surgical, or critical care.
- Excellent observation, verbal, and written communication skills.
- Ability to work independently with minimal supervision.
