Job Title: Hospice Nurse - Venango

UPMC Home Health is seeking a compassionate and skilled Hospice Nurse to join its team in Venango and surrounding areas. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.

Responsibilities:
  • Attend compliance training and adhere to organizational standards of conduct, policies, and procedures.
  • Focus on customer service and continually strive to perform duties in a manner that results in optimal patient satisfaction.
  • Assign other nursing personnel, including LPNs and Aides, to assist in the delivery of services as ordered on the plan of care.
  • Supervise and teach other nursing personnel, including LPNs and Home Health Aides, as needed.
  • Identify deficits or exceptional performance issues of other nursing personnel and recommend solutions.
  • Coordinate case management activities within a multidisciplinary healthcare team to ensure appropriate delivery of services to patients.
  • Initiate, coordinate, and ensure ongoing communication with physicians and other members of the healthcare team.
  • Attend and participate in patient care conferences, team meetings, and other related meetings.
  • Perform duties and job responsibilities in a manner that coincides with the service management philosophy of UPMC Home Health Care towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Identify, develop, and participate in process improvement opportunities within the home health agency to enhance the quality of services provided.
  • Provide direct patient care, evaluate outcomes, and adjust nursing care processes as indicated to ensure optimal patient care.
  • Initiate, coordinate, and evaluate patient and family-centered health teaching.
  • Establish the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with physician orders and patient needs.
  • Advises and consults with family and other agency personnel, as appropriate.
  • Completes documentation, including visit assessment, OASIS, 485, and Verbal Orders, accurately and promptly in electronic documentation systems while in the patient's home and within the agency's and regulatory standards.
  • Exercises independent judgment in matters concerning emergent and non-emergent patient care needs and communicates with physicians.
  • Participates in weekend and on-call coverage rotation for the agency, as per BU guidelines.
Requirements:
  • Zero to two years of experience.
  • 6 months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
  • BSN preferred.
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
  • Ability to work productively and effectively within a complex environment, handle multiple/changing priorities, and specialized equipment.
  • Good clinical judgment with critical thinking, analytical, and problem-solving abilities required as related to various aspects of patient care.
  • Critical thinking skills necessary to exercise and lead others in application of the nursing process.
  • Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.
Licensure, Certifications, and Clearances:
  • UPMC approved national certification preferred.
  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
  • Graduate nurses must complete licensure examination within FOUR MONTHS of hire or within one year of graduation, whichever comes first.
  • Automotive Insurance.
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR).
  • Driver's License.
  • Registered Nurse (RN) OR Temporary Practice Permit (TPP).
  • Act 33.
  • Act 34.
  • OAPSA.
