Responsibilities- Provide direct nursing care to patients in accordance with established policies, procedures, and protocols of the healthcare organization.
- Implement and monitor patient care plans, ensuring that patients receive high-quality, patient-centered care.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care, working closely with physicians, nurses, and other healthcare professionals to ensure seamless care transitions.
- Note and carry out physician and nursing orders, ensuring that patients receive timely and effective care.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team, ensuring that patients receive the necessary support and resources for a smooth transition to home or other care settings.
- Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
