Catholic Health Service is seeking a highly skilled Home Infusion Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for providing pharmaceutical services to patients receiving infusion, specialty, or retail services.
Key Responsibilities- Medication Evaluation and Management: Evaluate physician medication orders for appropriateness, potential interactions, and stability. Make recommendations for therapy changes to physicians and healthcare practitioners as needed.
- Patient Database Management: Maintain accurate and up-to-date patient databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Clinical and Technical Support: Collaborate with the healthcare team to provide clinical and technical support as needed.
- Documentation and Communication: Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Regulatory Compliance: Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy.
- Education and Licensure: Bachelor of Science in Pharmacy or PharmD degree from an ACPE-accredited School of Pharmacy. Current licensure in the state of NY.
- Experience: One-year experience in home infusion, hospital, or managed care setting preferred. IV clinical experience preferred.
- Skills and Competencies: Strong clinical and analytical problem-solving skills. Outstanding telephone skills and etiquette. Skill in use of electronic data management and computer systems.
