About the Role

We are seeking a highly motivated and compassionate Community Resource Specialist to join our team at Community Health Partnership. As a key member of our Prism Community Collective, you will play a vital role in supporting survivors of the Club Q attack and the broader LGBTQ+ community.

Key Responsibilities
  • Develop and implement personalized care plans that empower survivors and align with our mission.
  • Connect survivors and their families to essential resources, including counseling, food, housing, and transportation.
  • Act as a liaison and trusted advocate for survivors and the LGBTQ+ community, minimizing barriers and ensuring access to necessary services.
  • Monitor service access and follow up with providers to ensure ongoing support.
  • Collaborate with our Senior Manager of Health Equity and LGBTQ+ Health Equity Manager to identify gaps in training and education for provider organizations.
Requirements
  • Bachelor's degree and/or four years' experience in social service or a related field.
  • At least two years of experience in direct service support, peer support, customer service, and/or program management.
  • Minimum two years working with the LGBTQ+ community and a demonstrated understanding of their unique challenges.
  • Independent problem-solver with the ability to navigate new roles and programs with confidence.
  • Experience working with community-based organizations, local providers, and resources.
What We Offer
  • A competitive salary range of $52,000 - $60,000 per year, depending on experience.
