Job Description

Overview:

Friend Health seeks a skilled Mental Health Case Manager to join our team. As a key member of our care coordination team, you will play a vital role in assessing clients' needs and connecting them with the necessary resources to achieve their goals.

Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop personalized treatment plans.
  • Collaborate with healthcare providers, community agencies, and family members to ensure seamless care coordination.
  • Develop and maintain relationships with community-based service providers to facilitate access to resources.
  • Maintain accurate and timely documentation of client interactions and services provided.
  • Participate in regular staff meetings and training sessions to enhance skills and stay updated on best practices.

Requirements:

  • Bachelor's degree in a related field (e.g., social work, psychology, human services).
  • Valid driver's license and reliable vehicle for travel (if applicable).
  • Experience working in mental health or a related field, preferably with case management experience.
  • Knowledge of ASAM criteria and DMS IV diagnostic tools.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to lift 15-25 pounds and stand for extended periods (if applicable).

Estimated Salary Range: $45,000 - $60,000 per year, depending on experience and qualifications.
