Our HouseCalls product offers annual clinical assessments in the comfort of patients' homes, helping identify and reduce health risks while coordinating follow-up care to improve overall well-being.

We're seeking a skilled Nurse Practitioner or Physician Assistant to join our team, providing flexible scheduling with no on-call, weekends, or holidays required. This role offers a unique opportunity to achieve work-life balance while making a meaningful impact on patients' lives.

Key Responsibilities:
  • Conduct in-home assessments, including medical history review, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses for care management and active medical management in furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate assessment findings to inform members' primary care physicians of potential gaps in care.
  • Recognize urgent and emergent situations and intervene accordingly.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date, with unencumbered licensure.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP or PA license in state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
