Job Summary:The Care Manager will be responsible for assessing the needs of clients, developing and implementing individualized care plans, and coordinating with healthcare providers and community resources to ensure seamless care transitions. This is an exciting opportunity to make a meaningful difference in the lives of our clients and their families.
Responsibilities:- Document all case activity, including outreach, consent development, and assessment, plan of care development, client progress, disenrollment activities, and/or continuity of care activities.
- Manage a caseload of 20-25 individuals and actively pursue referrals for the program.
- Collaborate with clients, families, and service providers to complete the Child and Adolescent Needs and Strengths Assessment (CANS-NY) and develop a comprehensive, client-centered Plan of Care.
- Provide referrals to community-based programs as requested/needed and follow up to ensure that services are appropriate and meeting the client's needs.
- Assess clients for Home and Community-Based Services and provide referrals for services if approved.
- Provide community/home-based health home services to clients face-to-face minimally once/month (frequency dependent on billing requirements).
- Ensure clients are connected to and active with preventative medical/dental services.
- Assure responsible transition of client service into and out of Health Home care, between child and adult health homes, and between inpatient and community care as appropriate.
- Participate in mandatory and optional training and weekly supervision.
- Bachelor's Degree in Human Services Field with a minimum of three years of experience in Social Services or 2 years as a Health Home Care Manager.
- Relevant experience in serving children and families in child welfare, crisis situations, behavioral health, primary health care, or social services.
- Experience coordinating and participating in team settings.
- Solid writing and verbal communication skills, with good documentation skills.
- Ability to work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities.
- Ability to work in an effective and focused manner when self-directed and beyond immediate oversight.
- Ability to deliver family-driven, youth-guided services.
- Knowledge of community resources and counseling/social work practices with high-risk populations.
- Requires computer proficiency, including word processing skills, and the ability to provide culturally competent practice.
- Must have a valid driver's license that meets The Salvation Army insurance requirements and regular access to a vehicle to provide home visits and outreach services.
