You will serve as a clinical resource for your team, providing case management for designated patients and delivering high-quality patient care, including infusion therapy, medication administration, and nursing treatments. Your expertise will be utilized to provide independent nursing visits, triage, and advice, as well as consultation with practitioners and administrative leadership.
Key Responsibilities:- Coordinate patient flow and manage daily scheduling to optimize resource utilization and patient care.
- Provide clinical expertise and guidance to team members, ensuring high-quality patient care and outcomes.
- Develop and implement individualized care plans, incorporating patient education and health promotion strategies.
- Collaborate with multidisciplinary teams to coordinate care and address complex patient needs.
- Perform complex nursing procedures, including pulmonary rehab, anticoagulation management, and concurrent assessment of patients.
- Associate's degree in Nursing from an accredited program.
- Registered Nurse License (Washington) required at hire or Compact License.
- Basic Life Support certification required at hire.
- Excellent communication, problem-solving, leadership, and decision-making skills.
- Bachelor of Science in Nursing.
- Ambulatory care nursing experience.
- Primary nursing or case-management experience.
