Job Summary

The Licensed Practical Nurse (LPN) is a vital member of the Surgical Specialty Clinics team at the Veterans Affairs, Veterans Health Administration. As an extension of the Podiatry Nursing team, this role requires a high level of autonomy, critical thinking, and self-direction to provide exceptional patient care in the Ambulatory Care setting.

Responsibilities
  • Provide safe, timely, and individualized nursing care to patients in the Ambulatory Care setting.
  • Function independently in the clinic setting, managing a nursing podiatry clinic and responsible for patient flow under the general supervision of the Podiatrist.
  • Recommend footwear to protect feet from problems, ensuring low-level diabetic patients have two serviceable pairs of shoes for the year.
  • Evaluate shoes on each visit and recommend returning to the vendor if feet show signs of redness over bony prominences.
  • Incorporate information from foot screenings to determine if care needs are specific to foot care, including ankle brachial indexes (ABI) or vascular studies ordered by the Podiatrist.
  • Use special pressure-relieving devices to protect patients' feet from harm and pad feet from reddened areas.
  • Perform wound cleansing, dressing changes, and educate patients on proper use of creams, wound cleansers, or antiseptics.
  • Debride toenails by cutting along the natural line of the nail, using mycotic nippers as needed.
  • Evaluate patients with prior low-level foot wounds for proper healing, changing dressings as appropriate per VA protocol and notifying the PCP or Podiatrist if problems arise.
  • Determine the frequency of Podiatry nail care needed by the patient and schedule appointments accordingly.
  • Recognize potential urgent/emergent patient care situations and take appropriate action to quickly obtain a higher level of care for the patient.
Requirements
  • Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • Completion of at least one year of additional qualifying experience at the GS-06 level or equivalent, fully meeting all of the performance requirements for the GS-06 LPN/LVN.
  • Knowledge and skills necessary to care out more specialized patient care duties, including assignments in more complex inpatient and/or outpatient care settings.
  • Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery.
  • Ability to proactively assist the RN or MD/OD in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments and appropriately recording patient's response to medications/treatments received.
  • Knowledge and skill to promptly recognize potential urgent or emergent patient care situations.
  • Assists individuals or groups of patients and their families to take an active role in promoting health lifestyles for more positive outcomes.
  • Exercises skill in influencing and communicating with unusually difficult to care for or communicate with patients and/or family members who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability to understand or follow instructions.
  • Comprehensive practical nursing knowledge and skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care.
  • Ability to support the RN or MD/DO in monitoring the practice of unlicensed nursing or non-medical staff in patient care delivery.

  • Basic Life Support (BLS)
