Job Summary

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized care plans.
  • Provide emotional support and counseling to patients and families, addressing issues related to grief, loss, and change.
  • Collaborate with healthcare teams to develop and implement treatment strategies and discharge plans.
  • Participate in weekend and holiday coverage, as well as on-call rotations, to ensure seamless patient care.
  • Communicate effectively with patients, families, and healthcare teams, utilizing strong verbal and written skills.
Requirements
  • Master's Degree in Social Work (MSW) required.
  • Minimum two years of experience in acute care or clinic settings preferred.
  • Ability to communicate fluently in English and Spanish is highly desirable.
  • Strong problem-solving and organizational skills, with ability to prioritize tasks and manage multiple assignments.
  • Excellent interpersonal and communication skills, with ability to work effectively in a fast-paced environment.
