Job Summary:

Risewell Community Services is seeking a skilled Licensed Practical Nurse to join our team at the Wyandanch Certified Community Behavioral Health Clinic. As a member of our multidisciplinary treatment team, you will provide high-quality psychiatric, substance abuse, and medical services to patients in a clinic and community-based setting.

Key Responsibilities:
  • Provide initial and ongoing assessments in behavioral health, substance abuse, and physical health.
  • Implement Psychiatrist, NPP or PA medical orders, including ordering necessary bloodwork and urine screening.
  • Work alongside the Registered Nurse and take advisement to provide direct care, counseling, and education to patients and families.
  • Assist in crisis management and support other members of the team during absence.
  • Perform health services such as psychotropic medication treatment, health monitoring, and smoke cessation visits.
  • Provide medication management, education, and administration, as well as screen for medication side effects and monitor withdrawal symptoms.
  • Administer medication (IMs) as needed and follow medication policy and procedure.
  • Conduct all assigned/scheduled appointments/visits and update Outlook calendar to reflect scheduled appointments.
  • Meet with all clinic clients a minimum of quarterly for Health Monitoring and Smoke Cessation for positive smokers.
  • Ensure timely submission and accurate completion of all documentation to coincide with the appropriate comprehensiveness to justify the respected CPT code.
  • Complete treatment plans for assigned caseload and obtain prior authorizations for clients.
  • Share in the joint responsibility of collaborating during clinical and staff meetings to aid in the integrative process of creating a patient-centered, strength-based treatment plan.

Requirements:
  • Currently licensed as a Licensed Practical Nurse by the New York State Education Department.
  • A valid and clean NY State driver license.
  • Minimum 1 year of experience in a treatment or rehabilitation setting preferred.
  • Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
