About the Position

We are seeking a highly motivated and compassionate Veterans Support Specialist to join our team at U.S. VETS. As a key member of our organization, you will play a critical role in supporting the successful transition of military veterans and their families through the provision of housing, counseling, career development, and comprehensive support.

About the Job

The Veterans Support Specialist will be responsible for guiding veterans to achieve their highest level of independence. This includes assessing, planning, implementing, coordinating, monitoring, and evaluating the selection of services to optimally meet each client's needs. The specialist will work collaboratively with clients, community partners, and other stakeholders to ensure that all necessary resources are available to support the client's journey towards self-sufficiency.

Key Responsibilities

  • Care Coordination: Coordinate care with VA personnel and other community partners on behalf of clients to secure eligibility documentation, mental and physical health services, and other essential resources.
  • Case Management: Facilitate client intake process, compile client files, and maintain confidential client records in accordance with organizational standards.
  • Individual Action Plans: Collaborate with clients to develop Individual Action Plans (IAPs), which outline specific goals and objectives for achieving self-sufficiency.
  • Progress Monitoring: Meet with clients as specified by program requirements and document progress notes in Data/Assessment/Plan (DAP) format.
  • Home Visits: Conduct home visits to provide case management services and ensure the health and welfare of clients.
  • Quality Management: Assist in quality management activities, data collection, and preparing reports to measure the effectiveness of programs and services.

Additional Responsibilities

The Veterans Support Specialist may be assigned to various programs within U.S. VETS, including Transitional Housing, Permanent Housing, Workforce, and Supportive Services for Veteran Families. Additional responsibilities may include:

  • Program-Specific Duties: Focus on providing case management services that result in clients transitioning to permanent housing or obtaining and maintaining employment.
  • Rapid Rehousing: Conduct rapid rehousing and homeless prevention services for veterans and their families.
  • Housing Stability Planning: Work with clients to develop a Housing Stability Plan, which outlines specific goals and objectives for achieving stable housing.

Requirements

  • Education: Bachelor's degree required; degree in Social Work, Psychology, Counseling, or other related social service field preferred.
  • Experience: Two years of experience in social services or related field preferred; experience working with homeless and/or veterans preferred.
  • Skills: Strong oral and written communication skills; professional manner and appearance; ability to work independently and within a team; computer proficient in Microsoft Office and Internet.

Salary Estimate: $45,000 - $55,000 per year based on location and experience.
