- Documentation: Document all care provided accurately and timely in accordance with agency policies and procedures, ensuring seamless communication and care coordination.
- Interdisciplinary Collaboration: Collaborate effectively with the interdisciplinary healthcare team, including nurses, therapists, and social workers, to ensure coordinated care delivery and optimal client outcomes.
- Professional Development: Participate in ongoing training and educational opportunities to enhance skills and knowledge in home health care practices, ensuring best practices and evidence-based care.
- Confidentiality and Ethics: Adhere to ethical and professional standards, maintaining confidentiality and respecting clients' rights and dignity, in accordance with agency policies and procedures.
- Infection Control: Follow infection control protocols and maintain a clean and hygienic environment to prevent the spread of illness and infection.
- Education: High school diploma or equivalent required.
- Training and Certification: Completion of a state-approved Home Health Aide training program preferred, with certification as a Home Health Aide (HHA).
- Experience: Previous experience in home care, long-term care, or related healthcare settings preferred.
- Skills and Qualities: Strong communication and interpersonal skills, with the ability to interact effectively with clients, families, and healthcare professionals, as well as a compassionate and empathetic attitude towards the needs of clients and their families.
- Physical Demands: Ability to lift and move clients safely, as needed, using proper body mechanics and assistive devices, with the ability to stand, walk, bend, stoop, and kneel for extended periods.
