Capital Area Health Network is seeking a skilled Licensed Practical Nurse (LPN) to join our Wellness Center team. Our Wellness Center provides comprehensive HIV and AIDS care to the Richmond community, and we are looking for a dedicated professional to help us deliver exceptional patient care.
Key Responsibilities:- Triage patients with medical emergencies and coordinate follow-up care with medical providers.
- Coordinate patient flow and ensure patient safety in the clinical setting.
- Travel to various medical offices to provide direct patient care and support.
- Conduct health assessments and re-evaluations, and provide patient education and support.
- Act as a patient advocate and consult with physicians as needed.
- Assist with the coordination of resources and development of policies and procedures.
- Participate in case conferences and quality assurance activities.
- Maintain patient files and ensure compliance with HIPAA regulations.
- Process referrals and understand insurance requirements.
- Valid Virginia driver's license and satisfactory driving record.
- Current CPR certification.
- Minimum three years of nursing experience, preferably in a community health or public health setting.
- Leadership skills and critical thinking abilities.
- Computer literacy and experience with computerized charting.
