LPN - Hospice Care Specialist

In this role, you will be responsible for providing compassionate visits to hospice patients in their homes or nursing facilities. You will document each visit in the Electronic Medical Record and report any changes to the patient's condition to the RN.
Key responsibilities include:
* Providing high-quality patient care and support
* Documenting patient interactions and updates in the Electronic Medical Record
* Collaborating with the RN to ensure seamless care
* Participating in backup On Call capacity 2-3 times per month and 2-3 holidays per year
Requirements:
* Graduate from an accredited Licensed Practical Nurse Program
* Minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience
* Strong PC skills and ability to navigate Electronic Medical Records
* Valid driver's license and reliable transportation for visits (except in Manhattan)
