We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide high-quality care to pediatric patients in their homes. As a Pediatric Home Care LPN, you will work closely with our team to deliver personalized care and support to our patients and their families.
Key Responsibilities:- Provide direct nursing care to pediatric patients in their homes, as prescribed by the physician.
- Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
- Work under the direct supervision of a Registered Nurse and/or designated supervisor.
- Perform nursing activities within the scope of practice, as defined by the state laws governing the practice of nursing.
- Document patient visits thoroughly and completely, following regulatory and payer requirements.
- Participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
