The estimated salary for this position is between $60,000 - $85,000 per year, depending on experience and qualifications.

Job Description:

Care Coordination:

As a Patient Care Coordinator, you will be responsible for tracking and organizing patient cases, working independently while building deep relationships with patients, support systems, and care partners.

Schedule and Flexibility:

You will typically work an 8-hour day, Monday through Friday, with weekends and holidays off, and have flexibility as you manage your own cases and schedule.

Required Skills and Qualifications:
  • Current driver's license, reliable transportation, and vehicle insurance
  • Associates Degree in Nursing (A.D.N) from an accredited school of nursing
  • Basic computer skills and proficiency in MS Word and Outlook
