Job Description
We are seeking a skilled and compassionate Residential Care Worker to join our team in the Philadelphia area. The ideal candidate will be responsible for providing direct support and supervision to individuals with intellectual developmental disabilities, ensuring their safety and well-being at all times.
The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and other healthcare professionals. A strong understanding of person-centered approaches and behavioral interventions is also essential.
Responsibilities
- Coordinate with program staff to implement residentially based interventions and assist individuals in achieving goals and outcomes as described in the ISP
- Facilitate healthy and socially acceptable behaviors while living in community-based sites
- Prompt or assist, as necessary, to ensure individuals participate in activities of daily living
- Engage individuals according to established supervision levels
- Assist in the process of identifying, developing, and implementing the ISP
- Implement goals from the individual's ISP
- Support and encourage individuals in movement towards greater independence
- Support individuals to successfully integrate into the community, such as teaching them transportation skills, connecting them to community activities and resources, and helping them identify gainful employment
- Complete and submit all program documentation/data collection forms
- Communicate with staff at shift change about the status of the individual
Requirements
- CNA/HHA preferred
- Excellent communication and interpersonal skills
- Ability to work effectively with individuals, families, and other healthcare professionals
- Strong understanding of person-centered approaches and behavioral interventions
