Our mission is to radically improve the health, happiness, and dignity of senior living residents. We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in a facility-based APP role. The primary purpose of this position is to administer Provider-level direct patient care services within the Post-Acute Care setting. Key responsibilities include conducting clinical interventional and episodic direct care visits, evaluating and treating residents for changes in condition, reviewing medications and orders, and coordinating care with collaborating physicians. The ideal candidate will possess a valid nurse practitioner or physician assistant license in the state of operations, current board certification in their field, and strong leadership, interpersonal, and problem-solving skills.
