Job Title: Nurse Practitioner PD

Overview:

The Advanced Practice Nurse plays a vital role in delivering high-quality patient care, utilizing a patient-centered coordinated care model. This involves demonstrating leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making.

As a key member of the healthcare team, the Nurse Practitioner works collaboratively with other professionals to assess, plan, and implement care for individuals with health and safety needs.

Responsibilities:

  • Provides medical assessments, treatments, and clearances for patients, team members, and external customers, adhering to specific company profiles and regulatory standards.
  • Delivers clinical care professionally, serving as a customer advocate and role model, and ensuring that practice accurately reflects NJ Board of Medical Examiners regulations, Code of Ethics, and current NJ Department of Health requirements.
  • Develops and implements appropriate plans of care, facilitates patient teaching and support programs, and evaluates current protocols and standards of care for translation of best practice into clinical practice.
  • Supports the development of evidenced-based practice protocols and standards of care, applies evidence and value-based practices to achieve positive outcomes for patients, and provides medication safely and effectively to promote effective recovery from on-the-job injury/illness.
  • Prescribes medications and/or recommends over-the-counter medications as appropriate, initiates laboratory tests, immunizations, and diagnostic test/procedures as necessary, and demonstrates competent clinical skills with treatment plans.
  • Collaborates with physicians and consults effectively with other members of the healthcare team, demonstrates effective communication skills, and provides accurate, systematic, and organized documentation of findings, recommendations, and education.
  • Participates in continuing education and maintains documentation, develops a personal plan of development in area of clinical expertise, and confers with leader on goals and objectives.
  • Provides culturally diverse and educationally appropriate education in relation to patient care/needs, serves as a clinical resource to nursing staff, patients, and other multidisciplinary team members, and promotes a collaborative work environment that supports the organizational mission, behavior expectations, and values.
  • Identifies ethical care issues and coordinates interdisciplinary collaborative meetings to resolve those issues, floats to various clinic locations as needed, and adheres to HMH Organizational competencies and standards of behavior.

Qualifications:

  • Graduate of an NLN/AACN accredited program in nursing, Masters Degree, and maintains clinical competencies through participating in continuing educational programs and clinical practice as mandated by Board of Nursing and national certification body.
  • Experience in relevant patient care setting or as regulations dictate, and valid Drivers license.

Licenses and Certifications:

  • NJ State Professional Registered Nurse License, Advanced Practice Nurse Certification (w/ Master's Degree), and AHA Basic Health Care Life Support HCP Certification.
