We are seeking a skilled and compassionate Registered Nurse to join our team in Solano County. As a Hospice Admissions Nurse, you will play a vital role in providing end-of-life care and support to patients and their families.
Key Responsibilities- Conduct comprehensive assessments of patients to determine their home care needs
- Develop and implement individualized care plans to meet the unique needs of each patient
- Provide education and support to patients and their families to promote independence and well-being
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Document patient progress and outcomes in a timely and accurate manner
- Current RN license in the State of California
- Minimum 1 year experience as a professional nurse within the last three years, preferably in public health, home care, or hospice nursing
- Current CPR certification
- Licensed driver with an insured automobile
We provide a competitive hourly wage of $60-$70, with negotiable salary and per-visit pay rates depending on experience. Our team is dedicated to providing compassionate and quality end-of-life care, and we are seeking a like-minded individual to join us in this mission.
