Hospice Admissions Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team in Solano County. As a Hospice Admissions Nurse, you will play a vital role in providing end-of-life care and support to patients and their families.

• Conduct comprehensive assessments of patients to determine their home care needs
• Develop and implement individualized care plans to meet the unique needs of each patient
• Provide education and support to patients and their families to promote independence and well-being
• Collaborate with interdisciplinary teams to ensure seamless care coordination
• Document patient progress and outcomes in a timely and accurate manner

• Current RN license in the State of California
• Minimum 1 year experience as a professional nurse within the last three years, preferably in public health, home care, or hospice nursing
• Current CPR certification
• Licensed driver with an insured automobile
