Nurse Practitioner

We are seeking a compassionate and skilled Registered Nurse to join our team at Heritage Trails Nursing and Rehabilitation. As a Registered Nurse, you will play a vital role in delivering high-quality care to our residents and patients.
Responsibilities:- Assume responsibility for the direct care of residents, including assessing their physical and emotional status and evaluating their need for continued or immediate care.
- Delegate responsibility for the direct care of residents to appropriate nursing personnel according to needs, physical layout of care areas, and staff ability.
- Be responsible for all phases of direct resident care, within the guidelines of the Nurse Practice Act.
- Ensure that nursing staff perform all restorative measures recommended in the plan of care and required by each resident's condition.
- Report all incidents of change in resident's condition to the Director of Nursing.
- Assist physicians on rounds and keep them informed of the status of each resident, changes in condition, and medical emergencies.
- Plan working schedule to allow for education, supervision, and evaluations of performance for all nursing personnel during the shift.
- Maintain completeness of resident charts and individual care plans, making sure entries are timely, informative, and descriptive of resident's condition.
- Write daily nursing notes on all skilled residents, and monthly summary notes on each resident according to posted schedule.
- Be responsible for timely execution of all physician orders, including laboratory testing, specialized therapy orders, and preparation and administration of all medications as indicated.
- Must have a valid RN license in good standing.
- Successful completion of a training and competency evaluation program required.
- Completion of orientation required by company policy.
- Health screening and TB test (if required) within two weeks of employment.
- Mandatory drug test within 90 days of hire.
- Must be free of criminal activity proven by a criminal background check.
- Show within three (3) days of hire satisfactory evidence of identity and eligibility for employment.
