Job Summary

We are seeking a skilled and compassionate Charge Nurse to join our Home Hospital team at Mass General Brigham. Key Responsibilities
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Provide direct patient care, including assessment, education, and treatment, in accordance with our organizational philosophy and standards of practice.
  • Collaborate with our multidisciplinary team to ensure timely and effective care coordination, including referrals to community resources and other healthcare services as needed.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Manage and prioritize multiple tasks and responsibilities, including patient scheduling, medication reconciliation, and documentation, in a fast-paced and dynamic environment.
Requirements
  • Current Massachusetts license as a Registered Nurse.
  • Minimum of one year of recent nursing experience in an acute or emergency setting, with a strong preference for home care experience.
  • Ability to work independently and as part of a team, with strong interpersonal and communication skills.
  • Strong organizational and time management skills, with the ability to prioritize multiple tasks and responsibilities.
  • Ability to work in a variety of settings, including patients' homes, and to adapt to changing circumstances and priorities.
