They serve as advocates for all those in our Northeast Ohio community, especially the most vulnerable.
Position Summary: Collaborate with healthcare professionals to deliver supportive medical care to clients regardless of their ability to pay. This involves providing nursing care in a mobile clinic or community health ambulatory setting, encompassing health promotion, patient and family support, health maintenance, disease management, coordination of care, and maintaining continuity of care.
Responsibilities:- Provide care at outreach and primary care clinics across Care Alliance sites.
- Support providers during examinations and treatments, including preparing exam rooms, handling instrumentation, and operating medical equipment.
- Perform triage/intake on all patients according to established procedures, expediting provider schedules and patient flow.
- Engage with the community and educate them about the services offered by Care Alliance Health Center.
- Prepare patients for visits, establish reasons for their visit, document chief complaints, and complete all necessary fields in EPIC.
- Review medication lists, allergies, pharmacy information, and immunizations.
- Conduct blood draws, lab work, and assist with women's health vaginal exams.
- Under the supervision of a Clinical Outreach provider, perform basic health assessments, vaccinations, injectables, and procedures.
- Properly dispose of packages, containers, materials, biohazards, and expired/wasted medications in designated containers.
- Perform basic health screenings and report findings to management or providers.
- Implement appropriate components of the plan of care under the direction of the healthcare team. Ensure all necessary point-of-care tests (POCT) are completed and documented in the Electronic Medical Record (EMR) before provider visits.
- Demonstrate familiarity with sterile techniques and setting up a sterile field.
- Collaborate effectively with other Care Alliance health professionals to facilitate a team approach to patient management.
- Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures; comply with legal, HRSA, and OSHA regulations in collaboration with the healthcare team.
