Key Responsibilities:
  • Train patients in home hemodialysis or peritoneal dialysis, ensuring they understand the treatment process and can perform it safely and effectively.
  • Conduct home visits to assess the patient's environment and provide ongoing support to ensure continuity of care.
  • Communicate patient assessment results, reassessment, and ongoing monitoring to physicians, team members, and other stakeholders as needed.
  • Document all nursing services in the Electronic Medical Record, including training sessions, routine and non-routine interactions, and phone conversations.
  • Participate in infection control monitoring, implementation, and recording as requested.
  • Be familiar with emergency equipment and procedures, and communicate regularly with patients on emergency preparedness.
  • Ensure clinic and patient supply inventory and usage are appropriate for patient prescriptions and place orders in a timely manner.
  • Continuously seek to improve patient and quality outcomes through tracking and trending of program and patient performance, using critical thinking skills and root cause analysis.
  • Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to responsible physicians or physician extenders.
  • Complete and document monthly review of patient medication profiles, and administer medications as ordered by physicians.
  • Perform on-call nursing services, nights and weekends, on a rotational basis as needed or assigned.
  • Be flexible with staffing locations and hours to accommodate patient and USRC home program needs.
Requirements:
  • Knowledge of federal, state, local laws and regulations, including healthcare professionals' practice act requirements as they pertain to home dialysis programs and patient care requirements.
  • Ability to train patients and/or care partners in self-care peritoneal dialysis or hemodialysis, meeting federal and state regulations governing Registered Nurse qualifications.
  • Ability to conduct home visits to assess the patient's environment and provide ongoing support.
