Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at AltaStaff. As a Nurse Practitioner, you will be responsible for reviewing and making recommendations on consumer assessments for long term care services and supports. You will maintain current knowledge of regulatory changes pertaining to quality and validity of assessments and assess cases presented, utilizing clinical competencies to review and validate assessment outcomes. You will work collaboratively to monitor and address clinical quality and gaps in care, provide clinical and educational assistance to support project initiatives, and demonstrate communication skills necessary to interpret age-specific responses. You will also participate in performance, improvement, and quality assurance activities and related meetings, and provide clinical and educational support to meet program goals. You will meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position. You will perform other duties as may be assigned by Management.
Required Qualifications:
Current NY Nurse Practitioner Licensure, Proof of Board of Certification, Proof of Malpractice Insurance showing professional liability coverage of 1M per incident/3M agg, 2+ years practicing experience as a Nurse Practitioner, Computer literacy and the ability to navigate new and emerging technology, Must meet BYOD requirements, Highly developed written and oral communication skills; strong interpersonal skills, Ability to work in a fast-paced and changing environment with proven ability to work with community groups.
Required Vaccinations and Records:
Upon credentialing all candidates will need the following or willingness for the following vaccinations. Physical exam signed and dated by a physician within the last year, PPD or QuantiFeron (within 1 year) or chest X-ray with Questionnaires (every 5 years), TDAP within 10 years, Quantitative Titers for MMR & Varicella, Hepatitis B vaccination schedule or Declination Form Also Accepted, Proof of Covid19 Vaccine, Proof of Flu Vaccine.
