Job SummaryWe're seeking a skilled Registered Nurse to join our care team as a Nurse Care Coordinator. As a key member of our team, you'll play a vital role in evaluating patients, creating care plans, and communicating with patients, families, and the care team.

Responsibilities- Evaluate patients and create personalized care plans
- Communicate effectively with patients, families, and the care team
- Collaborate with interdisciplinary teams to ensure high-quality care
- Provide emotional support and comfort to patients and their families
- Participate in ongoing education and training to stay up-to-date on best practices
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
