Description The Registered Nurse is responsible for providing nursing services for the Integrated Care Clinic and other programs, as assigned. Provides direct psychiatric and general medical nursing care for patients. Provides daily nursing oversight of Licensed Practical Nurses. Reports to the Nursing Programs Manager or Medical Director as required. Work is performed in clinical and office environments. Requires managing a number of competing work priorities in a time-restricted environment. Works with a challenging population of patients, often from diverse backgrounds, with varying degrees of life stress and comorbid medical and psychiatric conditions. Includes providing care to patients with co-occurring developmental disabilities and substance use disorders. EXAMPLES OF DUTIES: Participates in the efficient provision of daily nursing service delivery in the Integrated Care Clinic. Provides psychiatric and general medical nursing services including, but not limited to, medication administration. injections, phlebotomy, CLIA-waived testing, vital sign determinations, AIMS testing, mental status examinations, medication delivery, psychiatric triage, and other assessments as required. Oversee medical protocols relating to the prescribing of clozapine and coordinates blood work with designated laboratory. Communicates education, observations, significant data, and interventions to treatment team members via documentation and other methods (if necessary). Provide preventive healthcare education for chronic diseases (e.g., diabetes, obesity, hypertension, heart disease, mental illness, etc.), in both individual and group settings, including planning and implementing wellness groups and other educational activities that support patients in achieving their wellness goals. May include smoking `cessation, weight management, and other disease management interventions. Participates in peer record reviews to determine compliance with record-keeping requirements and standards of care. In accordance with Board of Pharmacy regulations governing Controlled Substances Registrations (CSR) and Alternate Delivery Sites, participates in the ordering, receipt, storage, dispensing, security, and recordkeeping of medications. May serve as the Responsible Party for the Integrated Care Clinic Medication Room. Assure the security and safety of the Medication Room and its contents to include setting security alarm in the evening and making sure that the alarm is armed before leaving the unit. Inventory and requisition needed supplies for nursing tasks, to include all necessary personal protective equipment and equipment needed to perform nursing tasks. Participates in program team meetings and other required conferences. Provides education to patients individually and within group settings related to proactive and holistic needs in a wellness and recovery-modeled format. Collaborates with the interdisciplinary treatment team and community providers on an ongoing basis to facilitate integrated care. Completes lab slips and call-in prescriptions to pharmacies as authorized by I-Care physicians. Consults with physician and complete pre-authorization forms when required by insurance companies. Educates patients about their medications when delivering medications to them. Notify patient's physician whenever they report side effects and/or have other serious symptoms. Provides crisis counseling as needed. Complies with established legislation, mandates, clinic standards and Norfolk CSB requirements regarding patient services and documentation. Orients new employees to medical and nursing policies and procedures. Participates in staff development activities and in-service trainings. Acts as liaison between the physicians, nursing staff, and other agency staff. Completes required trainings and certifications by their due date. Performs other related duties, as required. PERFORMANCE RESPONSIBILITIES: Although each position has its own ,unique duties and responsibilities. The following areas apply to every employee of the Norfolk CSB. CSB Staff roles may include assisting other units at times to assist them with coverage needs, specialized assistance, or to cross-train. Staff hired into direct service positions (direct service, monitoring direct service, supervising direct service) can work in units other than their primary role to provide administrative or direct service activities within their skill set, education, and training. They may also serve in "voluntary" activities outside of the CSB's core services working with the public for community outreach events, service fairs, homeless outreach activities, homeless day shelters, neighborhood outreach events, trainings, etc. in a direct service, clinical, or administrative role. Staff who are responsible for providing assistance in emergency situations will be assigned to a role that is appropriate to their comfort level, skills, education, and experience. PERFORMANCE REQUIREMENTS: Knowledge: • Knowledge of nursing science and its application to persons with mental health, substance use, and co-occurring disorders. • General nursing skills (blood pressure, weight, BMI. injections, phlebotomy, wound care, obtaining medical histories, etc.) • Principles and practices of psychiatric and general medical nursing care, including crisis intervention. • AIMS (Abnormal Invountary Movement Scale) assessments. Perform AIMS examination and document results; knowledge of common movement disorders seen in psychiatric outpatient settings. • Human Rights regulations. • Familiarity with the diagnostic criteria included in the current DiagnoStic and Statistical Manual of Mental Disorders; signs and symptoms of mental disorders, including cooccurring substance use disorders and withdrawal syndromes. • Community mental health programs and available community resources. • Licensure requirements, applicable state and federal standards, and programmatic policies and procedures. • Confidentiality of protected health information, including HIPAA and 42 CFR Part 2. • Current pharmacotherapy for psychiatric disorders, including medications, indications for use, standard dosing ranges, routes of adminsitration, adverse reactions and side effects, contraindications, and signs and symptoms of toxicity. • Crisis management and intervention. • Knowledge of case management principles. • Knowledge of computers and common software programs used in the medical office environment, including Microsoft Word, Excel, Outlook, and PowerPoint. Skills: • Coordinating, monitoring, assessing, and evaluating patient needs. • Collaborating with other care providers for services outside of the CSB. • Crisis prevention, intervention, and management. • Liaison with community resources. • Advocacy. • Boundary and limit setting. • Facilitate individual/group education. • Administering medications, including oral and topical administration, as well as intramuscular, intradermal, and subcutaneous injections. • Venous phlebotomy, fingersticks, and urine collection. • Maintenance of patient records in accordance with all regulatory standards and policies/procedures, including timely documentation and indivdualized and fully integrated service plans. • Demonstrate positive interpersonal communication and effective listening skills. • Establish and maintain effective working relationships with peers, supervisors, other staff, patients, and their families. • Function as a member of an interdisciplinary team. • Manage and prioritize multiple work tasks. • Demonstrate adherence to strict confidentilaity and ethical standards. • Apply agency policies and institute remediation and progressive disciplinary action, as necessary. EDUCATION/EXPERIENCE: Current and unrestricted Virginia Registered Nurse license (or licensed in one of the Nurse Licensure Compact States). BSN preferred. Minimum of one year of experience working with patients with severe mental illness, co-occurring substance use disorders, and co-morbid general medical conditions, preferably in an integrated psychiatric and primary care outpatient setting. ALTERNATIVE TO MINIMUM QUALIFICATIONS: An LPN who has been employed with the City for over one year, and then attains an RN license, would be able to use their prior service as an LPN to count towards the one year experience requirement CERTIFICATE/LICENSE: Current unrestricted Virginia Registered Nurse license. Possession of a valid driver's license and an acceptable driving record as reported by the DMV are required for persons using personal or agency vehicles in the performance of their duties,. Must be certified or certifiable in Basic Life Support and First Aid training.
